The Engagement Specialist serves as the central wellness point of contact for clients in the New Vista healthcare system. Operating in the local clinic(s), this position ensures coordination of care team members and provides linkages to health and wellness services. Serving as the client’s advocate and working to overcome obstacles to care, this position ensures the client’s voice is central. This position requires a general knowledge of the healthcare system and a proactive approach to learning about ever-expanding services within healthcare, benefit assistance and basic health screening processes. The Engagement Specialist thinks outside the box and finds ways to connect clients to the right care, ensuring a critical “no wrong door” approach - that any client is provided a universal gateway to enter any New Vista service. The Engagement Specialist is a positive team player that promotes a culture of inclusion and participation.
Required Education And Experience
- High School graduate or equivalent
- Minimum of 6 months experience in healthcare or customer service
- Valid driver’s license, proof of auto insurance and reliable transportation
Preferred Education And Experience
- Experience providing case management services
- CMA or bachelor’s degree in Psychology, Social Work, Public Health or related field
- Experience in integrated care service (e.g., both behavioral and physical healthcare)
Essential Functions

- Ensure clients receive services (virtual or in-person) in a timely and effective manner identifying and removing barriers to access
- Facilitate communication between clients, family members and the care team to ensure client satisfaction and quality of care
- Evaluate social determinants of health and link clients to resources such as housing supports, food, basic needs, financial assistance, and employment resources
- Intervene when appointments are missed to determine reason, barriers to access and identify any desires for care to be adjusted
- Identify and links clients to proper services that address those who have limited English proficiency, are deaf or hard of hearing, or who require age-related or cognitive accommodations
- Ensure continuity of care between internal and external providers, support transitions or adjustments to care and provide linkages to community resources
- Advocate with providers and payors for proper healthcare
- With a whole health lens, determine creative solutions to barriers to care directly or through facilitating connections to internal or external resources for issues such as transportation, housing and wellness
- Become familiar with the network of medical providers in the area and how to apply and access benefits
- Track appointments, medication, and other wellness needs for clients on caseload as needed
- Connect with the care team and help ensure clients do not fall through the cracks using creativity, teamwork and a whole health approach
- Cross train with other aspects of the Call Center, including Helpline and National Suicide Prevention Lifeline/988
