Position Title: Population Health Coordinator
Reports to: Chief Medical Officer
Location: Primarily Remote Work from Home
Work in collaboration with: Chief Medical Officer and Chief Product Officer internally. Will also work closely with office staff at customer sites - primarily Practice Administrators, Medical Assistants, Nurse Care Managers, and front office staff.
Position Summary:
The Population Health Coordinator works to manage automated and manual outreach campaigns with the Chief Medical Officer. This position offers growth opportunities, including the opportunity to grow into a Team Lead position. You will also be involved in iterating on our outreach efforts and gathering feedback from patients and customers. The Population Health Coordinator is dedicated to providing support to the Chief Product Officer and Chief Medical Officer to facilitate and execute patient outreach campaigns.
Responsibilities:
- Monitor outbound outreach campaigns
- Respond to inbound responses, requests, and questions
- Make outbound phone calls as part of the outreach campaigns
- Schedule patient appointments
- Manage patient outreach, care gap, and quality metric lists
- Work with various EHRs and population health technology systems to pull reports on patients and care gaps
- Assist with basic documentation in our databases as well as in customer electronic health records
- Collaborate with leadership team to iterate and innovate!
Location Details:
- Initial onboarding may be onsite around Boston or virtual
- Much of the work will be remote work from home
- Occasional travel into primary care practices
- We are primarily looking for candidates in the New England area
Ideal work experience:
- *Population health management (REQUIRED) - e.g. experience helping patients get preventive and diagnostic care. For example, as a Population Health Manager or Patient Outreach Coordinator for a population health program.
- Primary care practices - e.g. experience working within or in collaboration with primary care practices in a role that supports patients. For example, as a scheduler, medical assistant, office manager, care manager, case manager, or referral coordinator
- Technology - e.g. experience and comfort working with various healthcare electronic health records and communication technologies
